Advanced ​Care ​Management ​provides ​you ​with ​the ​training ​and ​skill ​set ​you ​need ​to ​effectively ​utilize ​i2iTracks ​to ​improve ​clinical ​quality ​outcomes ​and ​manage ​populations ​to ​better ​health ​in ​a ​proactive ​manner. ​ ​This ​class ​will ​help ​you ​to ​achieve ​Patient ​Centered ​Medical ​Home ​(PCMH), ​Meaningful ​Use, ​Pay-for-Performance ​and ​other ​incentivized ​Quality ​Improvement ​initiatives. ​ ​If ​you ​loved ​our ​i2iTracks ​Super ​User ​Training ​Class ​or ​if ​you're ​looking ​to ​create ​new ​i2iTracks ​experts ​at ​your ​organization, ​i2iTracks ​Power ​User ​Training ​is ​right ​for ​you!

During ​this ​four ​day ​advanced ​training, ​you ​will:

* ​Prepare ​reports, ​perform ​searches ​and ​analyze ​data ​to ​deliver ​better ​care
* ​Learn ​techniques ​to ​advance ​your ​use ​of ​i2iTracks ​to ​provide ​proactive ​care
* ​Pull ​the ​data ​required ​to ​satisfy ​PCMH, ​Meaningful ​Use, ​Pay-for-Performance ​and ​other ​program ​requirements
* ​Grow ​the ​value ​of ​your ​i2iTracks ​investment ​by ​identifying ​new ​uses ​and ​spreading ​applications ​across ​the ​care ​team

Prerequisites: ​ ​6 ​months ​of ​daily ​i2iTracks ​usage ​OR ​completion ​of ​i2iTracks ​101. ​ ​Participants ​must ​have ​basic ​knowledge ​of ​iPHA ​Reports ​and ​Dashboards.

The ​cost ​is ​$2,000.00 ​per ​participant ​and ​space ​is ​limited!
